Forms of Payment

I do not take insurance at this time however I can provide an invoice that will meet requirements for reimbursement from insurance if it is a covered benefit.

I do accept cash, check, HSA, FSA, Visa, Mastercard, Discover, American Express.

Client Services Cost

$130 per appointment

Sliding fee scale available: $80-$130

Initial Appointment

This is a 60-90 minute session to explore and assess your current whole life behaviors including:

-Personal and family health behaviors.

-Goals and desired outcomes.

-Review of current medical treatment, medication, and labs.

-Exploration of relevant lifestyle behaviors such as sleep, stress coping, and social connections.

-Address specific questions and concerns you have.

You will leave this session with tailored map of how and what we will use to achieve your goals or desired outcomes. Education on where you are in your journey and targeted actions to move forward. 

 

Follow-up Appointment

In order to provide the support, tools, and individual discussion to move forward on your journey, we will co-decide on a follow up pattern that works best for you. These can be 60, 45, or 25 minutes depending on what level of support you need.
